How Often Should You Penetration Test? The Real Answer

The traditional answer to this question has been reassuringly simple: once a year. Annual penetration testing became the default cadence, enshrined in compliance frameworks and security best practices dating back to an era when software shipped on CD-ROMs and infrastructure changes required purchase orders.

That answer made sense when it was formulated. If your application only changed a few times a year, annual testing could reasonably capture your security posture. The findings from January's test would still be largely relevant in December.

The problem is that almost no organization operates that way anymore.

The Math Doesn't Work

Consider a typical modern development team. They deploy to production multiple times per week—sometimes multiple times per day. Each deployment potentially introduces new code, new functionality, new attack surface. Over the course of a year, that's hundreds or thousands of changes to the application.

Annual penetration testing examines one snapshot of an application that exists in hundreds of versions throughout the year. The test captures vulnerabilities present on the specific day testing occurs, but says nothing about issues introduced the week before or the month after. You're measuring a moving target with a stationary instrument.

The gap is even more pronounced for infrastructure. Cloud environments scale dynamically. New services spin up in response to business needs. Configuration changes happen constantly. The external attack surface you tested in Q1 may look substantially different by Q3.

What Compliance Actually Requires

Many organizations test annually because a compliance framework tells them to. PCI DSS requires annual penetration testing. SOC 2 expects it. Various industry regulations have similar requirements.

But here's what's often missed: these frameworks establish minimums, not best practices. PCI DSS requires annual testing because it was written when annual testing represented a reasonable baseline. The standard explicitly encourages more frequent testing for organizations with significant change activity. Treating the compliance minimum as the security optimum misunderstands the purpose of these frameworks.

There's also the question of what happens when compliance and security diverge. If your application changes weekly but you test annually, you're compliant. You're also potentially exposed for 51 weeks out of every 52. The checkbox is checked, but the risk remains.

Trigger-Based Testing

A more sophisticated approach ties testing frequency to change activity rather than the calendar. Major releases get tested. Significant infrastructure changes get tested. New features that handle sensitive data get tested. The cadence varies based on what's actually happening in your environment.

This model is better than pure calendar-based testing, but it has its own limitations. Someone has to decide what constitutes a "significant" change. Development teams under deadline pressure have incentives to classify changes as minor even when they're not. And coordinating with external testing firms for every meaningful change introduces delays that conflict with deployment velocity.

There's also the question of what gets missed between triggers. Not all vulnerabilities result from recent changes. Sometimes testers find issues that have been present for years, lurking in code that no one has looked at closely. A trigger-based approach might never examine stable functionality that happens to contain long-standing vulnerabilities.

The Continuous Alternative

The fundamental tension here is between the depth that penetration testing provides and the frequency that modern development requires. Traditional testing is deep but infrequent. Automated scanning is frequent but shallow. Neither adequately addresses an environment that changes constantly and faces threats that evolve continuously.

What's needed is testing that combines depth and frequency—that examines business logic and chains vulnerabilities like a skilled penetration tester, but operates continuously rather than periodically.

The question of how often to penetration test assumes a model where testing is an event. In that model, the answer is "as often as you can afford and coordinate." Making the Transition

If you're currently testing annually, moving directly to continuous testing might seem like a large jump. A reasonable intermediate step is quarterly testing, which significantly reduces the gap between assessments while remaining manageable from a coordination standpoint. Some organizations test monthly for their most critical applications.

But these intermediate steps are exactly that—intermediate. They reduce the gap without eliminating it. For organizations serious about security in an environment of continuous deployment, continuous testing is the end state. The question isn't whether to get there, but how quickly.
